Benefits

All full-time employees are eligible to participate in the following plans on the 1st of the month following 60 days of employment.  Below is a brief description of each plan available.

Blue Cross Blue Shield Medical Plan

  • Large Provider Network available all over the U.S.
  • Deductibles consist of $850/per person
  • Prescription co-pays are $12/generic, $60/formulary, $80/non-formulary. 

Dental Plan

  • 100% Coverage for preventative and diagnostic care

  • 80% coverage on basic, endodontic, periodontal, and oral surgery service
  • 50% coverage on major restorative and prosthetic services
  • 50% coverage on orthodontic services

Vision Plan

Life Insurance and AD&D
These benefits are provided at no expense to the employee.  Each of these policies covers the employee at 1x their basic annual earnings (max of $200,000). We also cover the spouse at $5000 and each child at $2000.

Short-Term Disability
This benefit is also provided to you at no cost.  In a case where you become disabled (non-occupational illness/injury) and unable to work, disability insurance kicks in and provides 50% of your basic weekly wages up to 12 weeks.

Long-Term Disability
This company-paid benefit provides you 60% of your basic monthly earnings (calculated off your base salary with a maximum of $6000).  LTD benefits will apply after being absent 90 days due to an injury/accident or illness.

These plans allow you to use pre-tax dollars for both medical and dependent care.

Employee Assistance Program
This is a free and strictly confidential benefit that provides assessment and short-term counseling to employees, their spouses, and their dependents.

401(K) and Roth Plans
On the 1st of the quarter following 6 months of employment, you will be eligible to participate in our 401(k) and Roth retirement plans where you have the opportunity to receive 100% company match on the first 3% that you defer and 50% match on the next 2% you defer.

Time-Off Bank
Employees begin accruing TOB for paid-time off purposes immediately. TOB is accrued based on years of service. TOB hours can be carried over from year-to-year until reaching a maximum of 240 hours.

Direct Deposit
As an added benefit to all employees, payroll checks may be directly deposited into employee's bank account(s). Employees must sign authorizations if they wish to take advantage of this option.
